Value EPO Highlights

With four medical plans available and costs changing in every category, it’s more important than ever to review the plan choices below and use the Comparison Tool on the Forum to help you decide which is best for you.
The first two present a comparison of three options: Community Care Signature HMO; Community Care Value EPO; and Community Core PPO. The final two present costs for the Community High Deductible Health Plan (HDHP).

Eligible dependents
You can enroll your eligible dependents in medical, dental, vision, supplemental life insurance, long-term disability and long-term care coverage. You must submit a copy of the following legal documentation to add or enroll new dependents:
- County-issued marriage certificate or a state certificate of domestic partnership.
- County-issued birth certificate for a dependent child (married or unmarried) through age 25.
- When a dependent reaches age 26, coverage continues through the end of the month.
You must submit the required documentation to the HR Benefits Dept. by the deadline on your new employee enrollment event.
Life events
Certain changes in your life situation may qualify as a major life event. You may be eligible for a special enrollment period allowing you to enroll, or opt out of health insurance outside of Open Enrollment.
The following are examples of a qualifying life event: getting married or divorced, having a baby or adopting a child, or losing existing health coverage. Changes based on a Major Life Event must be submitted within 30 days of the event occurring, and must be consistent with your reported life event. For example, if you have a baby, you may add your new baby to the plan, but cannot add anyone else.
