Please remember that the health care providers you used under your prior health plan (under a Continuity of Care contract), may not be in-network providers under the health plans offered by Community. This means that if you elect to continue receiving services from non-network providers, Community health plans may pay benefits at a lower level than you received under your prior plan.

Also, if you enroll in the Community Core PPO Health Plan, you will have a new deductible and out-of-pocket expense limits.
